1. For “Contact Info” located at bottom of form, please provide your Skype IM. Your email address will be requested at a later stage in the booking process.
2. If you are more than 5 minutes late, without prior notice/communication, your appointment will be cancelled. You will have to wait until the following week to reschedule since only 1 appointment per week is allowed.
